Our home care specialty programs have been developed to assist patients, families and physicians treat a variety of diseases and conditions in the home environment.
We provide a range of home care services including senior care, nursing, physical, occupational, and speech therapy. In addition, our team of health care professionals has worked in conjunction with physicians to develop specialty programs to assist patients with managing conditions, surgeries, and diseases at home.
Often these specialty programs are an appealing alternative to “facility care” for patients as well as referral sources and payers because patients can avoid the hospital or stay there for the shortest period possible.
Joint Replacement Recovery & Orthopedic Services
Orthopedic surgeons trust us to help their patients recover quickly after total joint replacement (hips and knees). Commonly referred to as “ortho protocol,” our team uses the following approach. First, our nurse (RN) will visit the patient, evaluate the home for safety concerns, assess the surgical incision, and educate about medication and pain management. Within 48 hours (or less!), physical therapy is initiated and a plan of care is established. Then for the next 2-3 weeks, therapy continues every 1-2 days. Our team will update the surgeon on the patient’s progress, and we will prepare the patient for outpatient therapy by confirming the first appointment.
We also provide therapeutic management for arthritis, bursitis, osteoporosis, fractures, and other orthopedic conditions and surgeries.
Fall Prevention & Home Safety
Our team is trained in the latest research and approaches to fall risk and prevention. We will evaluate each patient’s home and make recommendations to enhance living safely and independently. Fall prevention also includes an assessment of medications, compliance with indications, as well as therapy interventions for gait, balance, and mobility.
Dementia & Alzheimer’s Care
Our Director of Nursing is certified in an approach to dementia care called, “Positive Approach to Brain Change” by Teepa Snow (link), and our in-home staff members are trained in this approach. Our team members understand the progression of dementia-related conditions, and they learn how to gently and methodically help those with dementia symptoms and behaviors.
Pediatric & Behavioral Health
We employ a family-centered approach to helping medically fragile children, so they can enjoy a safe, healthy, and fulfilling childhood. Our caregivers have pediatric experience, as well as experience dealing with emotional, behavioral, learning, or developmental issues that may require unique guidance or extra support.
Cardiac Care, Vascular Care, & Heart Failure
For those with Congestive Heart Failure (CHF), high blood pressure, cardiomyopathy, coronary artery disease, stroke, heart attack, and other cardiac and vascular diseases, our nurses assess chest pain, respiratory difficulties, and wounds to provide appropriate care. Our team educates patients on symptoms, treatments, and medication management to reduce hospitalizations and ER visits. Patients learn healthy heart living, self-care after surgeries, training on functional energy conservation techniques, and knowledge about medications. We then work in collaboration with physicians to keep cardiac patients safe and healthy at home using necessary physician-ordered care, assessment, monitoring, testing, and therapies.
Independent & Assisted Living Facilities
Our home health care team is accustomed to working across the continuum of care, so when patients leave the hospital or skilled nursing facility, they have the specialized care they need to keep them in their home setting, whether that’s an independent living situation or an assisted living facility. Our nurses, therapists and paraprofessionals are experienced across care settings, and have experience working with facility administrators to coordinate and deliver home care services to their residents. Our team can work to bolster, support, or complement any facility’s staff for medication management, therapy, or assistance with hygiene. Our goal is the same across the continuum of care – help the patient/resident stay safely in their home environment for as long as possible.
